What is integrated care?

Within the framework of integrated care, your doctor/therapist and outpatient psychiatric/psychosomatic specialist services provide you with joint support. They coordinate the treatment strategy in regular meetings and take care of your needs - also in crisis situations. You will be assigned an experienced, personal reference therapist, usually a psychiatrically/psychosomatically trained and experienced professional, who will accompany and support you on your way. The specialist is the contact person and the guide within the treatment team - if you need him/her, he/she is there for you. Your reference therapist is closely networked with your doctor and/or therapist and helps you with all questions concerning your treatment. Through this close network, you will receive timely help and information about necessary treatment options and ways to improve your condition and current situation. If necessary, you can get appointments with him/her in a timely manner. A telephone service is also available to further optimise your contact options. Your advantages: You can receive care in your familiar home environment. If needed, also in crisis situations. A crisis service is available to you and your relatives at any time of the day or night. Help is provided immediately, professionally and flexibly.
